I am new to CCS.
I have a MySQL database with 5 tables.
I used the application builder to create forms for 2 tables.
After spending a lot of time fine tuning the 2 forms, I
realized that I need to add a 3rd form.
This 3rd form is from the same database.
How do I add this 3rd form to my project?
I would like this 3rd form to:
a. Inherit the same theme as the first 2 forms.
b. Become just another page under the current set of pages.
How do I do this?

Right click on the pages section of your project, then create a new page (a blank new page). Select "Add record form" from the forms toolbox menu. Right click on the form you have just created and select 'Edit form layout', then choose the theme you want for it. In the properties section of your form, edit the data source property and set it with the table that you want for it.
I hope this help you. It will be a good idea to check the available tutorials that explain all this stuff.

Thanks for the tips.
I have been reading the tutorials.
They have this sort of information in different places
but not in a single spot.
(Unless, I have missed something.)
I did not know all the things I needed to do until I find out as I move forward.
