CCS ADD Action to On Change Event - Error:Form name is required.

When I try to add a Submit Form action to a List Box in a Grid on a Form the
following error msg is displayed, but there isn't a place to specify the
Form name:
Error:Form name is required.
Error:Error occurred while generating the "MeetingSchedule" page. (1)
