OK I have a support ticket open I have some issues so last night I let support know I would do a full reformat

My Specs
Windows 7 Ultimate 64
i7 3690x
32 gig ddr 3
2x 3 TB hard drive 64mb cache 6gps sata3

I did a reformat did windows updates then installed ccs5 (5.0.0.16254)
if i try to use the html template in builder I get an error but if i use the default vertical no problem.

Also if I use the panel on a form the form dont show even if yes is selected
one of my main issues is that if i create a login form it renames the first box login1
i right click the box and rename it to username it changes everything to username except for the events.php I have to edit it manualy has anyone else had these problems?? is there a pack I need to install?
