Editable Grid overwrites records

I have had a weird issue happen twice on two different applications on different databases. When a user updated a record from an editable grid, it updated every record in the database with the modifid fields. Fortunately, I had backups and I restored the tables. Has anyone else seen this happen? Can you tell me how to prevent it from happening? Thanks.
